Becoming a patient is easy – complete our online enrollment forms and our office will reach out to schedule your first appointment. 

Scheduling is based on geography. If you have a special circumstance, please include them in your New Patient Forms.

We are a mobile Primary care practice that delivers Primary Care services in private homes and senior living communities.

We use Nurse Practitioners and Physician Assistants, supervised by a board-certified Family Medicine physician.

In most cases, we operate as a patient’s PCP. However, we can also serve as a secondary provider, providing acute care when your regular doctor is unavailable.

We provide physical exams, disease management, medication management, neurocognitive testing, competency evaluations, minor medical procedures, diagnostic testing and lab work, all performed in the safety of our patient’s home.

Yes, your insurance covers the cost of your care. We accept all major, local insurances.

If you are admitted to the hospital, our team works closely with you to help transition you back home upon discharge.

You can schedule an appointment by calling our office or sending us an appointment request via our website or the patient portal.

As you age and your needs become greater, so does the frequency in which we visit. Based on your initial evaluation, a plan of care is created, and follow-ups are scheduled accordingly. Most patients are seen regularly every 4-6 weeks.

Visits are performed during the week, Monday through Friday.

Yes, our office regularly coordinates with specialists, home health agencies and other providers.

We are trained and competent in managing all types of medications. Whether it is routine coumadin management or durable medical equipment, our team can help coordinate most every medical need.
